MA Notary Certificates
Massachusetts General Laws Chapter 222, Section 15 lists the official notarial acts a notary may perform and provides mandatory certificate wording for acknowledgments, jurats, signature witnessings, and copy certifications. These statutory forms must be used whenever a notary performs those acts unless another law, court rule, court form, or filing requirement specifies a different certificate.

The statute also explains how to notarize signatures by mark and how a notary may sign for a physically unable principal, both of which still require the notary to complete the act using an acknowledgment, jurat, or signature‑witnessing certificate. Importantly, the law allows alternate certificate forms in two situations:
  1. when a court, state law, federal law, or regulation requires or permits a different form, including Land Court rules; and

  2. when a document will be filed, recorded, or governed by the laws of another state and includes that state’s certificate wording.
Even in those cases, the Massachusetts statutory forms may still be used unless the other law expressly prohibits substitutes.

Massachusetts Notary Public
Acknowledgment Certificate
Used when someone signs a document voluntarily.
​The certificate confirms:

✔️ The signer personally appeared before the notary
✔️ The notary verified their identity
✔️ The signer acknowledged signing the document willingly

Notary Jurat
Jurat Certificate
Used for sworn statements, affidavits, and forms that require an oath.
​The certificate confirms:

✔️ The signer appeared in person
✔️ The notary verified their identity
✔️ The signer signed in front of the notary
✔️ The signer swore or affirmed that the document is true

Signature Witness
Signature Witnessing Certificate
Used when the notary’s role is simply to witness the signature.
The certificate confirms:

✔️ The signer appeared in person
✔️ The notary verified their identity
✔️ The signer signed in the notary’s presence (No oath is involved.)

Copy Certification by Notary Public
Copy Certification Certificate
Used when a notary certifies that a photocopy is a true and accurate copy of an original document.
​The certificate confirms:

✔️ The notary personally made the copy
✔️ The copy is complete and unaltered
✔️ The original document was presented
