​How to Request an Apostille or Certification
Requesting a Massachusetts Apostille or Certification is straightforward. Follow these steps:
  1. Complete the Massachusetts Apostille Request Form
    (The form opens as a PDF.)


  2. Prepare Your Original Document(s)
    Ensure each document is an original and has been properly notarized or certified.


  3. Include a Pre‑Paid Return Envelope
    Any major carrier is acceptable: FedEx, UPS, USPS, or DHL.


  4. Mail Your Packet To:
